Automation bias is a key issue for artificial intelligence adoption. This can cause otherwise knowledgeable users to make serious, even obvious, mistakes. Organizational, technical, and educational leaders can reduce these biases through training, design, and processes. This paper examines automation bias and how to reduce it through three case studies: the Tesla autopilot accident, the Boeing and Airbus aviation accidents, and the Army and Navy air defense accidents.
